In-Home NP/PA Clinician - Part-Time Health Evaluator

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes medication/history review, a physical evaluation, and optional tests per the health plan. You may travel locally and conduct virtual visits as needed.
